A Walk to Care network consists of Contract Volunteers and Contract-for-fee Mental Health Providers, Financial Counselors, and Financial Planners.

Please send us your information and a Provider Specialist will assist you with completing a Provider Application and Agreement. Providers will be required to submit State Licensure or Registration with an Expiration Date, Resume, W9 Tax ID Form, and Minimum Liability Insurance Coverage of $1 million/$3 million along with a Provider Application and Agreement.
